Heat the milk in a saucepan without letting it boil.

Whisk the egg yolks and sugar together.

Temper this mixture with 1/3 of the milk while whisking.

Pour the mixture into the remaining milk, then add the chocolate and coffee extract.

Cook the cream until it coats the back of a spoon, stirring constantly with a wooden spatula (the cream is cooked when a streak left by your finger on the spatula does not wipe off).

Add the cold-water-soaked gelatine leaves.

Squeeze them before incorporating into the hot cream while whisking.

Strain through a fine sieve.
